Initial Visit and Payment
When you enter Suite 240, you will walk into a lobby without a receptionist. There is a black framed statement on the wall, “To let your therapist know you are in the lobby, turn on the switch next to their name”. You will see a panel of light switches underneath that sign with the names of therapists. Turn the switch next to my name on and leave it on. I will come and get you.
At your first visit, I will ask you to complete some paperwork prior to beginning your first session. Please save time by downloading my intake form and have it filled out and completed prior to coming to my office. I strongly encourage you to contact me in advance if you are unable to download the forms. I can mail them to you or I will have paperwork in the lobby for you to complete. We will not begin the session until I have the completed paperwork.
I do ask that you pay at the time of service, this includes co-payments and/or deductibles. If you are writing a check, make it payable to “Toni J. Horvath”. I also accept check, MasterCard, Visa, Venmo and Paypal. To maximize your therapy time, please have your check made out prior to your session.
My fee for individual therapy sessions is $150.00 for a 50-minute session.
Insurance
If this is a first time insurance appointment, please make sure that you have notified your insurance company with the date of your first visit. This will initiate your insurance company forwarding me your paperwork on a timely basis. After you have booked your initial appointment, I ask that you call my billing group, Nor Cal Medical Billing at 530-622-1017 They are open Monday - Friday 8:00 am to 5:00 pm. Please have your insurance card available so they can get your information accurately. It should only take a couple of minutes to complete the call.
Managed Care and EAP Companies
While Toni is a provider with the following companies, it is your responsibility to check with your provider to make sure that she is a part of your specific list of authorized, in-network providers. You will need to let your insurance company know that you have an appointment set PRIOR to your appointment. That will initiate any pre-authorization that may be required. If your insurance company provides you with an authorization code, please note it and provide the authorization number to my billing department when you call at 888/622-1017. Do ask what your co-payment is and be prepared to make payment at your session. You can pay with cash, check (made out to Toni Horvath), credit card (MasterCard or Visa), Venmo or PayPal.
