First Visit and Payment
At your first visit, I will ask you to complete some paperwork prior to beginning your first session. You may save time by downloading my intake form and have it filled out and completed prior to coming to my office. If you have not done so, I will have paperwork for you to fill out. I strongly encourage you to contact me in advance if you are unable to download the forms. We will not begin the session until I have the completed paperwork.
I do ask that you pay at the time of service, this includes co-payments and/or deductibles. If you are writing a check, make it payable to “Toni J. Horvath”. I also accept MasterCard and Visa. To maximize your therapy time, please have your check made out prior to your session.
My fee for individual therapy sessions is $120.00 for a 50-minute session.

Insurance
If this is a first time insurance appointment, please make sure that you have notified your insurance company with the date of your first visit. This will initiate your insurance company forwarding me your paperwork on a timely basis. After you have booked your initial appointment, I ask that you call my billing department at (888)622-1017. Please have your insurance card available so they can get your information accurately. It should only take a couple of minutes to complete the call.
